Discharge Medicines Review Service (DMRS)

Providing support to patients recently discharged from hospital. The free NHS Discharge Medicines Review Service is available at Dudley Taylor Pharmacies to those who live in Wales, whether you have just been discharged yourself, or if you are helping to care for someone who has just left hospital.

The Discharge Medicines Review Service (DMRS) ensures that changes to medicines are followed up in the community.

The aims of this service are:

  • To reduce the risk of medication errors and adverse drug events
  • To increase patient involvement in their own care
  • To help reduce medicines-related admissions to hospitals or care homes

There are two parts to the service:

1. Patient identification and medicines reconciliation. This involves checking to ensure that medicines prescribed in hospital, match medicines prescribed by the GP when the patient returns home. Any discrepancies found should be highlighted to the GP.

2. Support for adhering to medication. This is a discussion with the patient to establish how they use their medicines. This also ensures that any discrepancies identified in the first stage of the service have been rectified

  • Have you recently been discharged from hospital?
  • Have your Medicines changed as a result?
  • Would you like to know more about your medicines?
  • Are you having problems remembering to take all your medicines?
  • Or, Just want some friendly professional advice?

You may be entitled to the new NHS Discharge Medicines Service.

Bring in your discharge letter and speak with our Pharmacist.

Emergency Hormonal Contraception

The emergency contraception pill (sometimes known as the “morning after pill”) is available at Dudley Taylor Pharmacies without a prescription and if you qualify, free of charge.

If you have had unprotected sex, and are worried about getting pregnant, taking emergency hormonal contraception within 3-5 days can help prevent pregnancy.

The emergency contraceptive pill  can be used if you have had sex without using contraception, or if you have had sex but there was a mistake with your usual contraception (for example, a split condom or if you forgot to take your usual contraceptive pills).

The pill should be taken as soon as possible after unprotected sex - the earlier it is taken, the more effective it is.

If you think you need emergency hormonal contraceptive advice come and have a confidential and anonymous consultation with our Pharmacist, in the Pharmacy’s private consultation room, to make sure this is the right treatment for you.

If the emergency contraceptive pill is considered to be suitable, the Pharmacist can supply it to you without a prescription, and in most cases it will be free of charge. However, if our Pharmacist does not believe that this morning after pill is suitable for you, they will be able to direct you to either a doctor or clinic that can help you further.

Although it is effective and there are no serious side effects of using Emergency Contraception, this form of contraception should only be used in an emergency and not on a regular basis as a first choice method of contraception. At Dudley Taylor Pharmacies, we stock a full range of contraceptive products and our Pharmacy staff can advise you on the many options available to you.

Medicines Use Reviews (MUR)

Did you know that 50% of people are not taking their medicines correctly?

This is a medicine check-up service, which is useful if you regularly take several prescription medicines or are on medicines for a long-term illness. This confidential NHS service will help you to find out more about your medicines, identify any problems you may be having with them and help you to take your medicines to best effect.

Ask our Pharmacist about yours today.

  • Do you know why you are taking all your prescribed medicines?
  • Do you know how you can reduce side effects from your medicines?
  • Do you know how to get the most benefit from your prescribed medicines?
  • Do you sometimes forget to take your medicines?
  • Just want some friendly professional advice?

A Medicine Use Review (MUR) is not a full clinical review such as you would have with your doctor. It is a free and confidential service that gives you an opportunity to ask the pharmacist questions about your medicines; to help you understand your medicines better, and to identify any problems you might be having and suggest possible solutions.

The review takes place in our Private Consultation room at a time convenient to you, with our Pharmacists who have been specially trained to carry out this FREE NHS Service for our patients

Appointment are  not usually necessary, so call in to the Pharmacy today and find out more about the medicines you are taking and how to get the most out of them.

New Medicine Service (NMS)

When you are prescribed a medicine to treat a long-term condition for the first time, the pharmacist will support you to use the medicine safely and to best effect. We offer a Free NHS service for our patients taking some medications.

The New Medicine Service is a free NHS service, which we offer to patients who are receiving their first prescription for a medicine to treat any of the following long-term conditions:

  • Asthma, or lung conditions such as chronic bronchitis and emphysema
  • Type 2 diabetes
  • High blood pressure
  • Conditions where you take a medicine to control the way your blood clots.

The service aims to help you understand your condition better and get the most out of your newly prescribed medicine.
Between 30% and 50% of prescribed medicines are not taken as recommended. This means that a lot of medicines are wasted or are not as effective as they could be.

The service will:

  • Help you to find out more about the new medicine you are taking
  • Help to sort out any problems you are having with your new medicine
  • Give you a chance to ask our Pharmacist questions about your medicine and discuss any concerns you might have
  • Help to improve the effectiveness of your new medicine, for example, there may be an easier or better way to take it
  • Help you to understand more about your condition, and better able to manage it.
  • Help you feel more in control of your health and how to get the most out of your medicine.

This free NHS Service is provided in a private area within the pharmacy or if you prefer, you could choose to have the discussion over the telephone, and usually takes place a week or so after you have been issued with your new medicine (s).

Your pharmacist will ask you questions about how you are getting on with your new medicine, find out if you are having any problems and give you any information and support you need. You may have concerns or questions that you want to ask. You can ask anything at all about your new medicine.

Your pharmacist will arrange a follow-up discussion with you 14 to 21 days after the initial consultation, so you are able to talk about how things are going with your medicine and ask for more advice if you need it.
